Who can refer?
Any person, with permission from the individuals who will receive our support, can make a referral to our program. Referrals for our service can come directly from potential clients, their family and friends, healthcare professionals, Local Health Integrated Network (LHIN) Case Coordinators and community leaders.
Why refer?
We can make a difference. You can refer to us for assistance with emotional support, practical information, caregiver respite, bereavement support. Our team of volunteers lead by the Hospice Outreach Regional Coordinator, are experienced, compassionate, flexible and all are educated in the fundamentals of palliative care. We aim to become a valuable member of your support team while facing the effects of a serious illness.
How to refer

Please fill out as much information as you can on the referral form. Don’t worry if you don’t have access to the answers to all the questions. Our Hospice Outreach Regional Coordinator can collect the rest.
